Acknowledgment of Risk

I, the undersigned parent or legal guardian, understand that participation in activities at Tiny Tides Playroom ("the Facility") involves inherent risks of injury, illness, property damage, and other hazards. I acknowledge that these risks may arise from, but are not limited to: running, climbing, jumping, crawling, sliding, and other physical activities; falls from play equipment or collisions with other children or adults; use of toys, sensory equipment, climbing structures, soft play features, ball pits, and interactive activities; choking hazards associated with food, drinks, toys, or personal items; exposure to germs, viruses, bacteria, and communicable illnesses; the actions or negligence of other participants, parents, or guests; and equipment malfunction or unforeseen conditions. I voluntarily choose to allow my child(ren) to participate in all activities and knowingly assume all risks associated with participation.

Release of Liability

In consideration of being permitted to enter and use Tiny Tides Playroom, I, on behalf of myself, my child(ren), my spouse, heirs, representatives, and assigns, hereby release, waive, discharge, and hold harmless Tiny Tides Playroom, its owners, employees, contractors, volunteers, agents, affiliates, and representatives from any and all claims and liabilities to the fullest extent permitted by Mississippi law.

Parental Supervision

Tiny Tides Playroom is not a daycare facility and does not provide child supervision services. Parents and guardians remain fully responsible for supervising children at all times. Children may never be left unattended.

Health and Medical Authorization

I certify that my child(ren) are physically capable of participating and authorize emergency medical treatment if necessary.

Sickness and Communicable Diseases

I acknowledge the risk of exposure to communicable illnesses and agree not to bring my child if they are sick.

Personal Property

Tiny Tides Playroom is not responsible for lost, stolen, or damaged personal belongings.

Rules and Code of Conduct

Follow all posted rules and instructions. Wear socks in designated play areas. Refrain from roughhousing or dangerous behavior.

Indemnification

I agree to indemnify and hold harmless Tiny Tides Playroom from claims arising from my child's actions.

Severability & Governing Law

If any provision is unenforceable, the remaining provisions shall remain in effect. This agreement shall be governed by Mississippi law.
